Ratification Of Grant Application With Minnesota Department Of Human Services For Residential Or Home And Community-Based Services To A Specialized Population, And Authorization To Accept Grant Funds, Execute Grant Agreement, And Amend 2024 Social Services Budget
end
PURPOSE/ACTION REQUESTED
Ratify grant application with Minnesota Department of Human Services (DHS) for residential or home and community-based services to a specialized population, and authorize acceptance of grant funds, execution of grant agreement, and amendment to 2024 Social Services Budget.
SUMMARY
DHS has funding available to support individuals approved to receive Residential or Home and Community-Based Services, but due to their complex support needs, are not able to promptly identify a community placement. Dakota County requested these emergency grant funds from DHS to support a prompt transition for an individual into their new community placement after their prior placement was issued a Temporary Immediate Suspension (TIS) by DHS due to significant licensing violations.
Dakota County had previously identified a new community-based provider, Snyder Healthcare Systems, that can better support their needs, but environmental modifications are needed before they can transition from the hospital into this community placement. Dakota County and Snyder Healthcare Systems are working to license the new Community Residential Setting. The DHS grant funds will be used for home modifications, fencing, and vehicle modifications to improve the safety of the home for both the individual and staff supporting him. These adaptations will reduce the use of emergency services such as police or hospitalization when individual engages in unsafe behaviors.
By Resolution 97-679 (November 4, 1997), the Dakota County Board of Commissioners authorized continuation of Community and Social Services in Emergency Situations if ratification approval is received within a reasonable time frame.
OUTCOMES
How well: Using the grant funds, Dakota County can support a prompt transition for an individual into their new community placement.
How much: The DHS grant funds will be used for home modifications, fencing, and vehicle modifications to improve the safety of the home for both the individual and staff supporting them.
Is anyone better off: These adaptations will reduce the use of emergency services such as police or hospitalization when individual engages in unsafe behaviors.
